Maintenace Director Or Maintenace Assistant of the Year
For Nursing Homes, Assisted Living, CCRC, Hospitals, Adult Day Care
Deadline for Submission is December 31

Please write a compelling letter why your maintenance director or assistant should be selected as the Maintenace Director or assistant of the year. Give examples of how he / she supports the recreation department and improves quality of life for your residents. Please include color photos.

Please provide biographical information about the nominee.

Please include: name of facility, address, phone number, email address, person nominating and title, 2 references from the facility, name of person being nominated and title.

Letters must be typed. Nominatons and pictures will not be returned.
Letters must be postmarked no later than December 31.

Winners will receive a $100.00 cash award and certificate. The winners name will also be posted on Alternative Solutions Web Stie.
